Eligible To Receive The Funeral Assistance: In the absence of a named beneficiary in Membership Form of the deceased member, the following rules apply:
- For married deceased members
- His/her surviving spouse
- His/her children
- His/her relatives up to Second degree of consanguinity
- Any person provided that the surviving spouse, if still living has acknowledge that the said person shouldered the funeral expenses
- For single deceased members
- His/her parent
- His/her relatives up to Second degree of consanguinity
- Any person provided that the surviving relative, if still living has acknowledge that the said person shouldered the funeral expenses.
- For widow/widower members
- His/her children
- His/her relatives up to Second degree of consanguinity
- Any person provided that the surviving relative, if still living, has acknowledge that the said person shouldered the funeral expenses.
Documentary Requirements:
If claimant is spouse:
- Marriage contract of member issued by the LCR
- Birth certificate of spouse issued by LCR or 2 valid IDs with signature
- Death certificate issued by the Local Civil registrar (LCR)
If claimant is child:
- Death certificate issued by the Local Civil registrar (LCR)
- Birth certificate of child issued by LCR or 2 valid IDs with signature
If claimant is other than the spouse:
- Death certificate issued by the Local Civil registrar (LCR)
- Birth certificate of spouse issued by LCR or 2 valid IDs with signature
- If legal relative is living, duly notarized waiver of the legal spouse in favor of the claimant.
